2	Being on time for your appointment (within 5 minutes) Good Brushing Nothing loose or broken Having an appointment during the month of your birthday Loosing a baby tooth Every “A” on your report card (maximum of 10 points per school year) Participating in our monthly online Treasure Hunt Contest
4	WEARING your ABO t-shirt to your appointment *Carrying your t-shirt in or putting it on at the office does not earn points
5	Sending a picture of you in your ABO t-shirt (maximum of 2 pictures per calendar year)
10	Welcome to our practice Having records the same day as your new patient exam Having a regular dental check up or cleaning (maximum 2 per year) (must have documentation - click here to download the certificate) Completing a requested dental treatment consisting of: permanent extractions, exposures, fiberotomy, frenectomy Win the Monthly drawing in our online Treasure Hunt Contest
20	Not missing any regular appointments during your entire treatment Having nothing broken or loose during treatment Referring an immediate family member (mom, dad, sister, brother)
50	Referring a friend

What You Need to Know
(Swipe N’ Smile Rules)

The program applies to current patients of Dr. Becker’s effective July 2008 (points are not retroactive)
You must have your swipe card at your appointment in order to earn points.
Points are awarded for regularly scheduled appointments (non-comfort/emergency).
If you lose your card, a new one can be issued to you for 5 points.
Points are non-transferable and gift cards are not redeemable for cash.
Your account must be current in order to redeem a prize.
All patients are given a Becker t-shirt at their bonding appointments. If you lose your t-shirt or it no longer fits, you may redeem 10 points for a new one or you can purchase one in the office for $5.00.
To redeem 10 points for your dental cleaning, please click here to download the certificate and take it into your dentist to be signed. Be sure to bring the certificate to your next appointment. (Also, your dentist will get entered into a drawing to win a lunch for his/her office)
